What is the theory behind this Assessing your stage of change worksheet?

The transtheoretical model of behavior change is used to assess an individual’s preparedness towards a new healthier behavior. These stages include; Precontemplation (unawareness regarding having a problem), Contemplation (awareness about the problem), Preparation (intention to make a change), Action (making change) and Maintenance (working to  prevent relapse.

How will the worksheet help?    

This worksheet will help the clients in assessing which stage of change they are in. It will help them have insight into their recovery process.
