What is the theory behind this worksheet?

A thought record is a document most often used in cognitive-behavioral therapy for clients to note down thoughts arising during periods of negative emotional states. 

In the initial stages, therapists concentrate on three basic questions to assist clients in grasping the link between their thoughts and emotions. When focus is placed on these initial three questions, the document is commonly referred to as a 3-column thought record.

How will the worksheet help?

The worksheet will help you write down basic information about the triggering event that caused you distress and accompanying thoughts and emotions. It will help you become aware of the impact of your thoughts on your mood and how by changing these thoughts you can change your emotional state.

How to use the worksheet?

Answer the three questions and think about the connection between the event that happened to you and the following thoughts and emotions. This worksheet is going to be especially useful in the beginning stages of the therapy where you can discuss it with your therapist.
