What is the theory behind this worksheet?

A thought record is a document most often used in cognitive-behavioral therapy for clients to note down thoughts arising during periods of negative emotional states. 

Since the worksheet consists of five questions regarding the client’s experience of a certain situation, the document is commonly referred to as a 5-column thought record.

How will the worksheet help?

This worksheet will help clients become aware of the connection between the triggering event, accompanying thoughts, and emotions, and it will provide them with the opportunity to consider alternative ways of thinking and behaving in similar situations.

How to use the worksheet?

Answer the five questions and think about the connection between the event that happened to you and the following thoughts and emotions. Also reflect about the alternative ways of thinking and behaving in similar situations in the future that might be more beneficial for you.
