What is the theory behind the 5 stages of change worksheet?

Carlo Di Clemente and James Prochaska created the Transtheoretical Model (TTM) of behavior change in 1977, which forms the basis of the five stages of change worksheet. 

These five stages; are precontemplation, contemplation, preparation, action, maintenance, and termination. 

The TTM model combines four essential elements with a temporal dimension to assist clients in understanding behavioral transformation. It is not a single approach to change.

By answering the questions for each stage, individuals can identify obstacles, and plan to make changes. It breaks the bad cycle into healthy behavior.
