How will this worksheet help?

This worksheet will help the counselor evaluate his/her thoughts, actions, decisions, socializations, preferences, and other activities related to the probability of experiencing burnout. By answering the questions, the counselor can assess how “alive” he/she is while practicing. 

How to use the worksheet? 

This worksheet is divided into three columns. The first column is situations related to the personal and professional life of a counselor. The second column contains a rating scale by which the counselor assesses the frequency of the situation occurring in his/her life. The last column is where the counselor makes an action plan if his/her answer on the rating scale is between 3 to 1.
