Agoraphobia can be explained as fear of a situation where a person feels difficult to escape. A person with agoraphobia is likely to avoid situations like being in open or closed places. Upon encountering such situations they might have physical symptoms CBT has been found to be effective in the treatment of agoraphobia. The worksheet has been designed for individuals to deal with their fears and deal with distress adequately 

How will the worksheet help?

The worksheet will help individuals identify their triggering thoughts and understand their exaggerated cognitions of thinking about the worst outcome. It will help them understand how thoughts can lead to fearful responses. They will be able to  communicate their expectations from therapy and create goals for the betterment    

How to use the worksheet?

The individual will first have to report the situation along with their thoughts emotions and behavior. Later they require to report their distress by using a rating scale and then they will have to report about how they faced their fear previously to challenge their cognitions. In last they will make collaborative goals with a therapist.
