Mental health is the state of a person’s cognitive, emotional, psychological, and social well-being. It is more than the absence of mental disorders and affects how we think, feel, and act.
Discharge planning is the development of a patient’s personalized plan to ensure the smooth transition of a patient from the hospital/ rehabilitation facility.
An effective discharge plan will include;
- Appointments for follow-up services
- Recovery plan
- Relapse prevention plan
- Discharge medication
- Educational awareness of before and post-treatment procedures
- Referrals to other needed services (if required)

- Cognitive Behavioral Therapy (CBT)
CBT is a type of psychotherapy used to help individuals identify negative thought patterns, learn how to regulate responses to anxiety-producing situations, and develop healthy coping strategies.

This therapy focuses on behavioral modification by reinforcing adaptive behaviors and extinguishing unwanted behavior. Additionally, this therapy helps transform an individual’s behavior to align with the desired life goal.
