Habit Reversal Training Worksheet (PDF)

What is the theory behind this Habit Reversal Training Worksheet?

For chronic motor and tic disorders, habit reversal techniques are used to reduce the intensity of the occurrence of these unhealthy or sometimes extremely harmful behaviors. Hair pulling, nail-biting, skin picking, thumb sucking or other tics-associated disorders are the target of this training.

How will this worksheet help?

With the help of this worksheet, you can note down the pattern of your unhealthy habits (such as nail-biting, hair-pulling, etc). You can keep a record of what are the behaviors, thoughts, feelings, and situations that lead to such behaviors. In this way, you will get a clear understanding of how to reduce this habit.

How to use the worksheet?

Think of situations where you felt the urge to perform behaviors or habits that you want to change. Now write every day when you feel the urge and maintain a record. It will also help you determine your progress if you are already taking habit reversal training.
