When there are conflicts and unfulfillment of roles in a family, problems with a child’s mental health start arising. Children are unable to communicate these problems. Yet, with proper probing and interventions, it is possible to know how their family members behave towards them.

This worksheet will help children in communicating what their family members do in the family. Either they are fulfilling their roles and responsibility or they are the cause of the problem.
How to use this worksheet?
To use this worksheet, the facilitator is required to ask the child the questions below. If any relationship mentioned is not present in the child’s family, write N/A. write the child’s responses in the given space.
