What is the theory behind the template?

Using the Global Assessment of Functioning (GAF) scale from the DSM-IV can assist you in giving a broad evaluation of where your client is regarding their mental health impact on their functioning.

In more recent versions of the DSM, the GAF has been replaced with the WHODAS 2.0 in order to provide a more detailed evaluation of a client’s functioning. 

If you feel that the GAF does not give you enough information or evaluation for your conceptualization or reflections, then using the WHODAS 2.0 is a good option.

How will the template help?

This template offers you an understanding of the GAF scale and a template tracker to use with your client over the intervention process.

You can make regular evaluations to track any progress made or to monitor any areas of concern.

How to use the template.

Work with your client to gather information that illuminates how they are functioning at that point in time. You may wish to gather this information and then complete the template without your client seeing their score if you fear that this may negatively impact your client’s well-being.

It is helpful to complete the first evaluation during the intake process.
