What is the theory behind this Addiction Recovery worksheet?
Addiction to drugs or alcohol is a recoverable condition. Over the years many research based interventions have been developed to rehabilitate people addicted to substances so that they can resume their normal lives. An effective recovery plan utilises medication, individual, group and family therapies with a particular focus on preventing lapse and relapse. All treatments aiming for behavioural change such as leaving an addiction must begin with the individual’s motivation to change.

How to use the worksheet?
Write about the negative and positive aspects associated with your substance use. Reflect on the ways your addiction has affected your daily functioning, interpersonal relations, personal goals and aspirations, mood and behaviour. Based on this analysis, write down at least 5 reasons that you want to leave your addiction.
