What is the theory behind this Child Behavior Worksheet- Behavioural Contract?

A behavioural contract is a written agreement between parents and child that specifies what behaviour is expected of them in return for a reward of their choosing. It is a technique used to modify unhelpful behavior in children and adolescents by encouraging them to work towards earning a certain reward. The contract can focus on a single behaviour or lay out a set of rules concerning multiple behaviours that have to be abided till a certain date. 

How will the worksheet help?

This worksheet will provide a basic template of behavioural contract that parents can use with their kids to help introduce new behaviours or modify existing unhelpful ones. Drawing up the behavioural contract has to be a collaborative process between the parents and the child so that the child is well aware of what needs to be done.
