What is the theory behind this Fitness goal worksheet? 

According to self-determination theory (SDT), an individual’s awareness of physical training is significant for increasing motivation for fitness goals. It is well established that our physical state is closely linked with our mental state. Therefore, achieving fitness goals will contribute to improved mental health states.

How will the worksheet help?

Research indicates that the mind and body are connected. Therefore, working on physical fitness will help in feeling physically fit along with feeling psychologically fit. This worksheet will help you evaluate your current health choices, and will help in making new ones.

How to use the worksheet?  

This worksheet comprises two kinds of fitness goals 1) Nutrition goals 2) Exercise goals. Nutrition goals can be related to adding or subtracting certain food groups/drinks in your diet. Exercise goals are related to physical exercise.
