What is the theory behind the template?

Informed consent is a key element in any therapeutic setting. Before intake information can be fully gathered, a therapy treatment plan or conceptualization can take place, and the client (or parents of a minor client) must give informed consent.

Elements that should be included are services offered, privacy and confidentiality, contact with the practitioner, etc.

How will the template help?

This template provides a basic informed consent agreement for a client (adult or child) to begin a therapeutic process. The template includes acknowledgement of POPIA and will be a good addition to the basic intake process and documentation.

It is important to note that this does not take the place of a contract, rather it aims to enhance it and ensure that the client understands all the terms and conditions of the commitment they are making.

How to use the template.

This template can be used as is, or it can be used as a guideline to tailor an informed consent form suited to your specific workplace.
