Trauma Worksheet for Adults

What is the theory behind the Trauma Worksheet for Adults?

Trauma is a strong negative emotional response to being exposed to an event that threatens one’s physical, psychological and emotional well-being. Everyone responds differently to trauma in their life. Some may have acute symptoms while others may continue to experience symptoms even after years of the traumatic event.

How will the worksheet help?

This worksheet will provide a list of common trauma symptoms that clients can use as a checklist to identify which symptoms apply to their trauma reaction. Narrowing down the specific type of symptoms experienced by the client can help the therapist formulate a tailored treatment plan.

How to use the worksheet?

Instruct the client to go through the given checklist of common signs and symptoms of trauma and mark the ones that apply to them. In the blank space they can also add any symptoms that are specific to them and any extra details that need to be mentioned about it.
