What is the theory behind this Treatment Planning Worksheet?

Cognitive Behavioral Therapy requires a collaborative relationship between the client and therapist so that they can come up with treatment goals together. To make it easier for clients to describe their problems and what they have in mind for therapy sessions, the therapist can use a treatment planning worksheet to know about the clients presenting problems and their expectations from therapy.  

How will the worksheet help? 

This worksheet will help therapists determine their client’s presenting problems and their hopes and expectations from therapy. It will not only help the therapist set treatment goals but also tailor the therapy specific to the needs of their clients. Responses can be further discussed before the treatment plan is finalised.

How to use the worksheet?

This worksheet is to be used in the very initial sessions so that the therapist can gain  a better understanding of their client’s expectations from therapy and their goals for their therapeutic process. Instruct the client to answer the questions as provided in the worksheet.
