How will this worksheet help?

This worksheet can be used by the client who succeeded in treatment and decides to pursue recovery. Beyond the confines of treatment, the client is left out on his own to face the world. This worksheet will serve as a guardrail to the choices the client makes relative to his quest for recovery.

How to use the worksheet?

This worksheet will be primarily used by the client in assessing the choices he is about to make or may have initially made. Ideally, the support of others is much welcome, however, to promote autonomy and independence, the final decision should be made by the client.
