During a group therapy session or program, there are elements of participation, confidentiality, and social communication that must be addressed during the intake process.
Adding this template can enhance the Contract of Service or Commitment Contract as it is specific to a group process. This template would ensure that all clients are well-informed and provide consent for the process.
How will the template help?
This template can be used in conjunction with other intake forms for consent or confidentiality. It does not replace the other documentation but rather acts as an enhancement to be used with group therapy.
How to use the template?
This template can be discussed with the clients in the group to ensure that they understand all that it entails. It could be a good idea to create a specific group agreement together with the clients regarding expectations and agreed-upon behaviors. This agreement can be attached to the template and recorded.
