What is the theory behind this Self-Care Assessment Worksheet (PDF)?

Self-care involves all activities and behaviours that enhance the health and well-being of an individual’s physical, spiritual, social and psychological self. It is critical for individuals with mental illness diagnosis or those who are prone to stress to prioritise self-care as a part of their coping strategies since its the first thing that is ignored despite its benefits. 

How will the worksheet help?

This worksheet can help therapists get information about how well their clients are engaging in self-care activities as discussed in therapy. It will also help clients assess the barriers that they have been facing in doing these activities and how they can overcome those barriers to ensure consistency. 

How to use the worksheet?

Instruct the client to note down the self care activities that they engage in over a period of two weeks. Then tell them to note down how often they engaged in that activity. Then they must also assess what barriers they faced and how they can overcome those barriers to make those self care activities a part of their regular routine.
