What is the theory behind the template?

When you work clinically as a mental health practitioner it is part of your best practice to attend supervision sessions. These might be clinical supervision with an expert or peer supervision with your colleagues.

During these sessions, you will be required to present a summary of a case you are working on. This template offers a guideline for the information that would be helpful to include in one of these presentations.

How will the template help?

This template offers an overall view of helpful information needed to present a case in supervision.

This template can also be helpful if you have a long-term client and wish to gain a summary reflection on their progress at the end of a certain period, for example at the end of the year.

How to use the template?

Complete the template with as much information and reflection as possible. You should be able to have a complete in-depth understanding of the client from the information provided.

Remember that this is a template and a guideline, you may wish to tailor it to your specific practice or the legalities required in your location.
